You shouldn't have to fill in any information here, but if no information is present these are the default settings for databases on an SQL server located on your computer:

„Next you should press the "Deploy Database" button. This will create the database on the server you specified in the previous step, using the database name that you specified as well. This may take a few moments. Please make sure to allow this process to finish before going any further"
Now you can test the connection by clicking the "Test Connection" button. The "Test Connection" button can also be used when moving databases between servers. „If the result of the connection test is „green" the system is fully configured and ready to use". Please use the users manual for any usage guidance.
If you get errors or even if you do not get errors and after restart you still can not connect to the database, your user (on the computer) does not have privileges to create database or alter one in the SQL Server.· Your user needs to have Connect and Execute rights on an existing database.· It is also wise to have dbcreator role for the user.
